A unique new feature has been added to the online appointment booking and prescription ordering service called 'Patient Access'. You can now request to act as a proxy for children, relatives and dependants that you care for using Patient Access.
This enables a parent, family member or carer to act on behalf of the patient with their access tailored accordingly.
Appointment booking, repeat medication requests and where applicable, access to medical records, can easily be accessed by the proxy once a relative or person has consented and been linked.
To apply for Proxy Access you will need to complete the following GRMC Proxy consent form and attend the surgery with two forms of ID, one of these must contain a photo. Acceptable documents include passports, photo driving licences and bank statements.
Visit the Patient Access website for further information.
